ย้อนกลับไปที่ ปี 2027 ก่อน Deus Ex ภาคแรก 25 ปี อดัม เจนเซ่น (Adam Jensen) อดีตหัวหน้าหน่วย SWAT ที่ได้รับว่าจ้างให้มารักษาความปลอดภัยและปกป้องความลับของบริษัท ชารีฟ อินดรัสทรี (Sarif Industries) ในห้องแล็ปทดลองพัฒนาอวัยวะเทียมลับสุดยอดของบริษัทที่ควบคุมดูแลโดย Dr.Megan Reed (ดอกเตอร์ เมแกน รี้ด)
แต่ก็เกิดเหตุการณ์ก่อการร้ายขึ้น โดย ชารีฟ อินดรัสทรี ได้ ถูกกลุ่มชายชุดดำลึกลับโจมตีที่ทำให้ อดัม ได้รับบาดเจ็บอย่างหนัก ทางรอดเดียวของเขาคือฝังอวัยวะจักรกลเทียมเข้าไปในร่างกาย ทางเลือกว่าก็คือ Nano-Augmentation หรือการฝังนาโนเทคโนโลยีลงในร่างกายแล้วให้ความสามารถพิเศษต่างๆ แต่ใน Deus Ex ภาค 3 นี้ Augmentation
จะไม่ใช่นาโนเทคใต้ผิวหนังที่ดูสะอาดตาอีกต่อไป คุณจะเริ่มเกมอย่างมนุษย์ธรรมดาๆ แต่เมื่อเนื้อเรื่องดำเนินไปเรื่อยๆ คุณจะถูกกดดันจนต้องดัดแปลงตนเองไปในที่สุด ด้วยชิ้นส่วนจักรกลที่ออกแบบให้ดูคล้ายอวัยวะเทียมในปัจจุบัน อย่างเช่นมือข้างใหม่ที่ทำด้วยเหล็กกล้าหุ้มแผ่นพลาสติกสีเนื้อจะมาแทนที่ มือเก่า ชิ้นส่วนคุณติดตั้งลงไปแต่ละชิ้นจะแทนที่เลือดเนื้อของคุณ และให้ความสามารถพิเศษใหม่ๆ พร้อมกับรูปลักษณ์ที่ผิดแปลกออกไปในเวลาเดียวกัน
