การต่อสู้กับสัตว์ประหลาดที่เรียกว่า Necromorph ในยุคอวกาศได้เริ่มขึ้นอีกครั้ง เมื่อนักวิศวกร Isaac Clarke ได้ตื่นขึ้น 3 ปีหลังจากเหตุการณ์อันแสนเลวร้ายและน่ากลัวบนยานขนาดยักษ์ Ishimura ที่ซึ่งมนุษย์ได้ถูกปลุกขึ้นให้ฟื้นจากความตายและกลับกลายเป็น Necromorph Isaac พบว่าตัวเองถูกตามหลอกหลอนโดยแฟนเก่า Nicole ที่เขาทิ้งให้ตายไปในเหตุการณ์ครั้งก่อนนั่นเอง เขาต้องพยายามดิ้นรนเอาชีวิตรอดจากฝันร้ายและหาคำตอบให้กับความสับสนอลหม่านซึ่งเกิดขึ้นบนยานลำใหม่ที่มีชื่อว่า The Spawn
