เหตุการณ์ภายในเกมจะเกิดขึ้น ณ โรงพยาบาลบ้าขนาดใหญ่แห่งหนึ่ง ที่ตั้งอยู่ห่างไกลผู้คนบนเทือกเขาโคโลราโด ในสหรัฐอเมริกา ซึ่งสถานที่บ้านพักผู้ป่วยทางจิตแห่งนี้ได้ถูกปล่อยให้รกร้างมาเป็นเวลานานหลายปี จนกระทั่งเมื่อไม่นานมานี้ได้ถูกเปิดใช้งานอีกครั้งโดยองค์กรลึกลับนาม “Murkoff Corporation” โดยอ้างเหตุผลว่าเพื่อการวิจัยและการกุศล, ซึ่งปฏิบัติการทุกอย่างภายในได้ถูกเก็บงำเป็นความลับจวบจนถึงปัจจุบัน , ผู้เล่นจะได้สวมบท “Miles Upshur” นักข่าวอิสระที่ได้ข้อมูลจากแหล่งข่าววงในเกี่ยวกับความลับของสถานที่แห่งนี้ จึงแอบแทรกซึมเข้ามาเพื่อทำข่าว แต่ก็ต้องพบเจอกับความจริงอันน่าสะพรึง ที่ซ่อนอยู่ภายในใจกลางหุบเขาโคโลราโด ซึ่งมีเรื่องราวผสมปนเปทั้งวิทยาศาสตร์ ศาสนา และเรื่องเหนือธรรมชาติที่มิอาจหาคำอธิบายได้
