เกม NBA 2K22 ทางทีมงานได้มีการปรับแต่งระบบการเล่นให้มีความเหมาะสมดุลมากขึ้นกว่าเดิม ด้วยการมาของระบบกลยุทธ์แนวรุกแบบใหม่ และระบบการป้องกันที่ได้รับการยกเครื่องใหม่ทั้งหมดเพื่อให้ประสบการณ์ในการแข่งขันมีความสนุกมากขึ้น เช่นเดียวกับระบบสกิลต่างๆ ทั้งการเลี้ยง, การชู้ต, การดังค์, และการทำอัลเลย์อูปส์ (alley-oops) ก็จะได้รับการปรับปรุงใหม่พร้อมกับระบบการบล็อกใหม่ และอื่นๆ อีกมากมาย

ซึ่งเกมก็จะมาทั้งการแข่งขัน NBA และ WNBA ที่ได้รับลิขสิทธิ์อย่างถูกต้อง และเกมยังมีโหมด MyTeam ที่ผู้เล่นสามารถสร้างทีมบาสเกตบอลของตัวเองได้ รวมไปถึงเกมก็ยังมีโหมด MyCareer ให้ผู้เล่นได้สมบทบาทเป็นนักบาสเกตบอลอีกด้วย

โดยในเกม NBA 2K21 ได้มีการเปิดตัวระบบซีซันเข้ามาในเกมที่เป็นระบบที่จะทำให้ผู้เล่นได้รับของรางวัลต่างๆ ไปใช้ใน MyTeam ได้ ซึ่งในปีนี้ระบบซีซันก็จะกลับมาอีกครั้งและก็จะมีบทบาทสำคัญมากกว่าเดิมทั้งใน MyTeam และ MyCareer รวมไปถึงโหมดการเล่น The W Online ซึ่งจะเป็นโหมดการเล่นสำหรับเครื่องเกมรุ่นใหม่โดยเฉพาะ
