The Last Stand: Aftermath เป็นเกมแนวเอาตัวรอด / Top-down Shooter ในโลกหลังยุคล่มสลายสไตล์ Roguelite จากผู้สร้าง The Last Stand: Union City และ Dead Zone ซึ่งผู้เล่นจะต้องรับบทเป็นผู้รอดชีวิตที่พลาดติดเชื้อ เราจะต้องออกสำรวจในโลกที่เต็มไปด้วยความผีดิบกินคน แต่อย่ายอมแพ้เพราะว่าก้าวต่อไปของเราอาจจะพาไปสู่เรื่องราวหรือความหวังใหม่ก็ได้ ผู้เล่นจะต้องตามหาน้ำมันเพื่อใช้เดินทางยังพื้นที่อื่นๆ และเก็บวัสดุมาคราฟต์อาวุธหรือไอเทม

ผู้เล่นต้องมีสติตลอดเวลาเพราะว่าเวลานี้เหล่าซอมบี้ยังเร่ร่อนกันเต็มถนน และเราเองก็เป็นแค่มนุษย์เดินดินธรรมดาแถมยังติดเชื้อด้วย ดังนั้นก่อนที่เราจะต้องตายด้วยพิษของไวรัส หรือถูกรุมทึ้งฉีกเนื้อก็ต้องเอาตัวรอดให้นานที่สุด โดยหลังจากตัวละครเสียชีวิตลง สเตตัส, ไอเทมและ Perk ต่างๆ จะถูกโอนต่อไปให้ตัวละครถัดไปทันที พร้อมกับออปชั่นเสริมที่สามารถปลดล็อคได้เพิ่มเติม
