Heart of Iron IV คือเกมแแนว Real-time Strategy ที่ให้เราได้ปกครองประเทศต่าง ๆ ของโลกในช่วงปี 1936 ที่ประเทศต่าง ๆ กำลังอยู่ในช่วงของการเปลี่ยนแปลงหลังสงครามโลกครั้งที่ 1 และเรานี่แหละผู้ที่จะกำหนดชะตาของโลกใบนี้
สิ่งที่ทำให้เกมนี้แตกต่างจากเกมสงครามโลกครั้งที่สองอื่น ๆ คือ ตัวเกมเน้นในการวางแผนและการใช้กลยุทธุ์มากกว่าที่เคย เพราะว่าในเกมนี้เราไม่ได้แค่สร้าง Unit แล้วออกไปรบกับชาวบ้าน แต่เราจะต้องผลิตอาวุธปรับแต่งกองทหาร วางแผนการรบที่ Status ทุกหน่วยมีค่า สามารถตัดสินการแพ้ชนะของสงครามได้เลย
ซึ่งความยากง่ายของเกมจะขึ้นอยู่กับประเทศที่เราเล่นเป็นหลัก โดยหากเราเล่นประเทศใหญ่อย่าง สหรัฐกอเมริกาหรือเยอรมนีก็จะเล่นง่ายกว่าประเทศอื่น ๆ เนื่องจากมีโรงงานในการผลิตอาวุธที่แยอะและช่องทางในการวิจัยเทคโนโลยีที่มากกว่า ในขณะที่ประเทศเล็ก ๆ มีทรัพยากรจำกัดหากวางแผนไม่ดีอาจจะพังได้ง่าย ๆ
นอกจากนี้ตัวเกมยังให้อิสระกับเราในการสร้างเนื้อเรื่อง เช่นเราสามารถที่จะเล่นอเมริกาแต่ไม่เข้าร่วมฝ่ายสัมพันธมิตรก็ได้ เล่นประเทศสยามแต่ประกาศสงครามกับญี่ปุ่นก็ได้เช่นกัน อยู่ที่เราว่าจะเลือกเล่นแบบไหน โดยตัวเกมเองยังมีระบบ National Focus ที่จะทำให้แนวทางการเล่นมีความหลากหลายยิ่งขึ้นกว่าเดิมในบางประเทศ
