เกมส์ Cities XL เริ่มต้นความสมจริงกันด้วยฉากแผนที่แบบ 3 มิติที่มีการจำลองอัตราส่วนของโลกจริงๆมาไว้ภายในเกมส์ รวมไปถึงสภาพภูมิประเทศในส่วน ต่างๆของโลกด้วย ซึ่งผู้เล่นสามารถเลือกบริเวณต่างๆของโลกจากแผนที่โลก ที่ต้องการจะสร้างเมืองได้ โดยมีหลายสถานที่จริงๆชื่อดังของโลกมากมาย ไม่ว่า จะเมืองดังอย่างนิวยอร์ค ลอนดอน ปารีส หรือแม้แต่เมืองแถบประเทศที่เป็นภูเขาสูงและทะเลทราย ทุกทวีปทั่วโลกผู้เล่นสามารถเลือกสร้างเมืองได้หมด แต่รู้ สึกจะยกเว้นทวีปแอนตาร์คติก เพราะคงไม่มีผู้ใดไปสร้างเมืองขั้วโลกใต้หนาวๆแบบนั้นแน่ รายละเอียดกราฟิกของภูมิประเทศต่างๆในแผนที่นั้นถูกออกแบบมา อย่างครบถ้วนและ สมจริงที่สุด ไม่ว่าจะเป็นชายหาด เอกเขา ภูเขา ยอดเขา หุบเขา แกรนแคนยอน เกาะ หมู่เกาะ น้ำตก ฯลฯ และสภาพภูมิอากาศและพืชพรรณ ธรรมชาติก็ทำออกมาให้เข้ากับฉากแผนที่ของประเทศ นั้นๆอีกด้วย และไม่ใช่แค่รายละเอียดของแผนที่ธรรมชาติเท่านั้นที่สมจริง บรรดาสิ่งก่อสร้างต่างๆก็มี ความสมจริงอย่างมากด้วย มีตึกหลากหลายประเภทกว่า 500 แบบ และบรรดาสถานที่ต่างๆมากมาย
