Sniper Elite 4″ เป็นผลงานเกมแนวซุ่มยิงภาคต่อของบ.Rebellion ที่คราวก่อนให้เราไปคลุกทราย คลุกฝุ่น ไล่เด็ดหัวนาซีที่แอฟริกามาแล้ว โดยในภาคนี้จะดำเนินเรื่องต่อจากเหตุการณ์ในเกมภาคสามที่วางจำหน่ายไปเมื่อปี 2014 โดยยังคงยึดฉากหลังสงครามโลกครั้งที่สอง เพียงแค่ย้ายสมรภูมิรบไปซุ่มยิงกันต่อที่ประเทศอิตาลี ในปีค.ศ. 1943 แทน
เมืองที่สวยงามภายใต้ไฟสงคราม มีเพียงคาร์ล แฟร์เบิร์น พลซุ่มยิงรูปหล่อเท่านั้นที่จะช่วยชาวเมืองให้รอดพ้นจากไฟสงคราม
เราจะเป็นพลซุ่มยิงหน้ามนคนเดิม คาร์ล แฟร์เบิร์น (Karl Fairburne) ตัวละครเอกประจำซีรีย์ ที่ต้องกลับมาจับปืนสไนเปอร์คู่ใจอีกครั้ง และร่วมรบเคียงบ่าเคียงไหล่ไปพร้อมกับเหล่าหนุ่มสาวชาวอิตาเลียนใจกล้าแห่งกองกำลังต่อต้าน เพื่อหยุดยั้งโค่นล้มลัทธิชาตินิยมฟาสซิสต์ของจอมเผด็จการชื่อก้องโลก เบนิโต มุสโสลินี (Benito Mussolini)

ทางตัวแทน Rebellion ออกมาตอบคำถามนี้ โดยระบุว่า ภารกิจในอิตาลีนั้นสำคัญไม่แพ้ภาคก่อนๆ เพราะถ้าภารกิจในอิตาลีล้มเหลว ก็จะไม่มีทั้ง ปฏิบัติการโอเวอร์ลอร์ด, การยกพลขึ้นบกที่นอร์มังดี หรือแม้กระทั่งชัยชนะในสงครามของฝ่ายสัมพันธมิตรในเวลาต่อมา
